This method connects and improves a care manager's efficiency to succeed in delivering higher touch care. We can save health plans and risk-bearing providers billions and mitigate the risk they take by putting focus on preventing avoidable costs in their highest risk populations. In helping patients to improve adherence to their care plans, we are able to drive down hospitalizations, readmissions, and complications and produce meaningful cost savings for our customers; often within 3 months in our program.
