Community Health Worker (Bilingual Spanish)

- $50,000–$55,000/year

What You'll Do:
- Conduct community outreach and health screenings
- Support Medicaid members and individuals with asthma or chronic health conditions
- Complete health assessments and questionnaires
- Conduct home visits (approximately 8 per month; some virtual)
- Connect community members with resources and health-related services
- Assist with environmental health initiatives and supply coordination
- Present educational workshops and community programs
- Maintain accurate records across multiple systems
Requirements:
- Fluent in Spanish and English (Required)
- Must live in Staten Island, NY or within a few mile radius.
- Passion for community health and helping underserved populations
- Ability to work independently and stay organized
- Comfortable with outreach, home visits, and public presentations
- Tech-savvy and able to learn multiple systems quickly
- Healthcare, social services, community outreach, or customer-facing experience preferred
