Position Summary:
The Medical Records Specialist compiles, processes, and maintains medical records of CCI Health Center patients in a manner consistent with medical, administrative, ethical, legal, and regulatory requirements of the health care system. The Medical Records Specialist acts as a liaison between the medical record vendor and CCI Health Services ensuring that document indexing, record management, and other requests are being handled appropriately and in according with any regulatory compliance requirements.
KEY FUNCTIONS AND RESPONSIBILITIES:
- Reviews medical records for completeness, accuracy, and compliance with regulations; notifies providers of record deficiencies; and tracks outstanding records.
- Manages incoming faxes, images/x-rays, and consult notes when routed by third-party medical record management vendor (GRM).
- Establishes and trains on processes related to document management within EHR, naming conventions, and any misses due to incorrect documentation/coding by the provider.
- Protecting patient confidentiality in compliance with HIPAA and other legal requirements, ensuring authorized access only.
- Assures appropriate medical release of information is documented in patient records for all insurances, legal organizations, and other identified entities.
- Provides medical record information by answering questions and requests of patients, providers, law firms, insurance companies, and government agencies.
- Resolves or clarifies codes or diagnosis with conflicting, missing, or unclear information by consulting with providers or others.
- Assists with patient demographic information, scanning documents, and handling faxed documents related to patient care.
- Identifies, compiles, abstracts, and codes patient data, using standard classification systems.
- Employees serving in a lead, senior or similarly designated position will be required to perform clinical duties daily.
- Ensure proper coding and indexing of medical records for easy retrieval.
- Verify the authenticity and authorization of record requests.
- Maintain the integrity of record filing systems.
- Other duties as assigned.
REQUIRED SKILLS:
- Minimum High School diploma with training/experience in the medical field required, an Associate of Applied Science degree in Health Information Management Systems preferred.
- A minimum of 3 years of relevant experience, preferred.
- Experience with medical billing and coding practices, preferred.
- Experience with eClinicalWorks, highly recommended.
