  • Pay: $60/hour first 90 days → $120,000 salary after ramp
  • Schedule: Minimum 32 hours/week availability

Deliver in-home primary care visits for adult patientsConduct physical exams and diagnose conditionsPrescribe medications and adjust treatment plansBuild personalized care plans for chronic conditionsCoordinate care with specialists and healthcare partnersComplete follow-ups after hospital discharge (TCM visits)Document visits and communicate with care team

Must-haves


MD, DO, Nurse Practitioner, or Physician Assistant licenseLicensed in AlabamaMaster’s degree in Nursing or Physician Assistant StudiesStrong clinical assessment and diagnosis skillsAuthorized to work in the United StatesComfortable working independently in patient homes

Experience in in-home or mobile healthcareWound care certification or experienceExperience with transitional care or chronic care management
