What You’ll Do
- Deliver longitudinal primary care
- Manage transitional care post-discharge
- Reduce hospital readmissions
- See patients where they live — private homes, assisted living, skilled nursing (~30%)
- Oversee care across a defined regional footprint
- Build real patient relationships
Must-Haves
- MD or DO
- Board Certified/Eligible IM or FM
- Active state license (or willing to obtain)
- Comfortable with community-based care delivery
- Eligible to work in the U.S.
Nice to Have
- Experience in home-based, ALF, or SNF settings
- Interest in regional leadership growth
