Care Navigator – Remote (Healthcare / Referral Coordination)


  • Role: Care Navigator
  • Location/Type: Remote • U.S. (Pacific Time preferred)
  • Pay: $21.50–$33.50/hour (DOE)
  • Schedule: Full-time • Standard business hours

Role Snapshot


This remote role is ideal for experienced Medical Assistants, Referral Coordinators, or Care Coordinators who enjoy closing care gaps and guiding patients through next steps.You’re the primary point of contact for patients with complex musculoskeletal conditions. You explain care plans, coordinate referrals, verify benefits, and make sure nothing gets missed. High ownership. Clear handoffs. No guessing.

What You’ll Do


  • Call patients within 1–2 business days after provider visits
  • Review care plans and explain next steps clearly
  • Verify insurance eligibility and estimate out-of-pocket costs
  • Obtain prior authorizations when required
  • Place referrals and coordinate scheduling with external providers
  • Follow up on appointments, tests, and outstanding items
  • Request and track outside medical records and results
  • Maintain local provider lists and referral relationships
  • Document all activity accurately in the EMR
  • Flag workflow gaps and suggest process improvements

Must-Haves


  • High school diploma or equivalent
  • Medical Assistant degree or similar non-licensed healthcare training
  • 2+ years as a Medical Assistant or Referral Coordinator
  • Strong experience with insurance eligibility and benefits breakdowns
  • Solid medical terminology knowledge
  • Comfortable working independently in a remote environment
  • Fluent written and spoken English
  • Reliable high-speed internet

Nice to Have


  • Spanish fluency
  • 4+ years MA or referral coordination experience
  • Musculoskeletal (MSK) care exposure
  • Prior remote or healthcare call-center experience
