We're seeking a dedicated and compassionate Community Health Coordinator to join our team and help connect vulnerable individuals with essential health and social services. This position recruits, onboards, supervises and coordinates the activities of Community Health Workers. The Coordinator facilitates relationships across the agency and with community partners that minimize barriers to care for clients. This role plays an integral part in ensuring that clients are able to access care and that services are delivered in a responsive and consistent manner.

 

What You'll Do:


  • Supervises the 421 Fallsway-based Community Health Workers staff, including management of their schedules and promotion of the services they provide. Provides discipline specific guidance to Community Health Workers staff at community sites to ensure a uniform approach and proper training across the agency.
  • Sets clear expectations, monitors outcomes, creates a culture of open communication, and helps team members solve complex problems through individual supervision and team meetings. Drives accountability through the delivery of high-quality care and accurately and timely documentation of all interventions and encounters according to agency policy.
  • Establishes key performance indicators (KPIs) for the community health programs and monitors progress towards intended outcomes. Collaborates with Director of Supportive Services and Director of Compliance to track and report programmatic outcomes for funders.
  • Facilitates, organizes, and assigns internal requests for community health engagement from a variety of sources, including but not limited to care teams.   
  • Partners with population health staff to increase community health presence on targeted health initiatives and performance improvement activities.  Supports the use of client registries that identify needs and guides community health workers in delivering effective health-promoting responses. 
  • Ensures Community Health Worker coverage for clinic navigation to help clients navigate the system and access the services they need within the clinic. 
  • Ensures that required screenings are completed in accordance with agency protocols and procedures.

Key Agency Responsibilities  


In addition to role responsibilities, each staff member of Health Care for the Homeless has the following responsibilities as a part of their employment: 

  • Models and reinforces the Health Care for the Homeless “core values” of dignity, authenticity, hope, justice, passion and balance
  • Actively participates in performance improvement activities and actively participates in advocacy activities that support the mission of Health Care for the Homeless
  • Performs other duties on an as-needed basis
  • Protects our clients’ personal health information by maintaining compliance with HIPAA and other relevant Health Care related IT security regulations

What You'll Bring: 


Formal Education and Training 


  • Bachelor’s degree in Community Health, Public Health, Human Services or related field required. 
  • A valid driver’s license and access to a personal vehicle required. A driver’s record check will be conducted pre-employment.
  • Bilingual Spanish-English highly preferred

Experience 


  • Four years of experience providing community health services to vulnerable individuals and/or connecting individuals to essential services including health care, mental health and addiction services
  • One year of supervisory experience required. Two years strongly preferred
