Role Snapshot
- RN Case Manager – Home Health
- $40/hr
- Full-time (36–40 hrs/week)
- Onsite • In-home visits across the DFW region

What You’ll Do
- Provide skilled nursing care in patients’ homes
- Follow clear care plans and clinical protocols
- Monitor vitals, medications, and medical equipment
- Document visits in Kantime EMR (modern system)
- Coordinate care with families and clinical team
- Support safe transitions and continuity of care
Must-Haves
- Active Texas RN license
- Available for 36–40 hrs/week
- Able to complete home visits in assigned counties
- Comfortable working in home health settings
Nice-to-Have
- Kantime EMR or EVV experience
- Prior home health experience
- Bilingual (Spanish)
