What You’ll Do
- Provide care in nursing and long-term care facilities
- Manage chronic conditions, perform full assessments
- Close care gaps (HEDIS and STAR measures)
- Lead care planning with facility teams
- Support and educate patients’ families
Must-Haves
- Active, unrestricted APN license in Michigan
- Board certified and Medicare credentialed
- 6+ months in skilled nursing or long-term care
- Solid with digital tools, team-based care
- Able to work onsite in Midland, MI

- Pay: $123K–$135K base + $15K bonus
