Job title: Family Assessment Worker- Bilingual
In this role you will:
- Establish trusting relationships with families, in consideration of families’ personal experiences and diverse cultural backgrounds.
- Meet with families in the hospital to engage families and gain referrals into the program. Enroll families into the Healthy Families program.
- Promote community awareness regarding Healthy Families program services.
- Develop positive relationships with community providers to secure referrals and provide support to families.
- Engage in face-to-face contact with community partners to provide presentations.
- Compose accurate and complete in-depth assessments, program intake documents, and data tracking regarding the status of screening/assessment activity, within appropriate timelines.
- Provide comprehensive data, recording-keeping and reporting of community outreach efforts.
What it takes:
- High school diploma or equivalent + 3 yrs experience in social services OR associates degree or higher in human services or related area.
- Bilingual Spanish/English.
- Strong communication and writing skills.
- Experience working with culturally and linguistically diverse populations.
- Knowledge of family dynamics and community resources is preferred.
- Attention to detail with the ability to perform assignments and job responsibilities efficiently, accurately, and on time.
- Proficiency in Microsoft Office and Windows
- Valid US state driver’s license
- Possess an insured & reliable vehicle and a valid AZ driver’s license.
- Valid Arizona Fingerprint Clearance Card - OR - must qualify for a valid Arizona Level One Fingerprint Clearance Card
- DES Criminal Affidavit Required
- AZ DHS Criminal History Affidavit Required
- FPC DPS Verification Required
