- Pay: $125,000–$142,000 + travel bonus
- Location: Travel across South Carolina • Onsite in skilled nursing facilities
- Schedule: Mon–Fri, days • No on-call • No weekends

What You’ll Do
- Own your patient panel across partnered SNFs
- Manage chronic conditions and complete full assessments
- Close care gaps (HEDIS quality measures)
- Lead care planning with your IDT
- Keep patients and families informed
- Document fast with modern tools
- Reduce avoidable hospital use
Must-Haves
- Active, unrestricted APN license in South Carolina
- Board certified + Medicare credentialed
- 6+ months SNF or long-term care experience
- Comfortable with digital tools
- Able to travel daily to multiple SC facilities
