What You’ll Do
- Provide compassionate care in skilled nursing and long-term care settings
- Manage chronic conditions & perform comprehensive assessments
- Close care gaps (HEDIS and STAR measures)
- Lead interdisciplinary care planning
- Keep families informed and supported
Must-Haves
- Active, unrestricted APN license in Indiana
- Board certified + Medicare credentialed
- 6+ months of experience in skilled nursing or long-term care
- Comfortable with digital tools & team-based care
- Able to work on-site in Versailles, IN
