- RN Case Manager – Home Health

- Onsite • Home visits across the DFW region

What You’ll Do
- Deliver in-home skilled nursing care
- Manage care plans with clear protocols
- Monitor vitals, meds, equipment
- Document in Kantime EMR (modern system)
- Coordinate with families and clinical team
- Support safe transitions and continuity of care
Must-Haves
- Active Texas RN license
- Able to work 36–40 hrs/week
- Willing to complete home visits in your county/neighboring counties
- Comfortable with home health environments
- Work authorization in the U.S.
Nice-to-Have
- Kantime EMR or EVV experience
- Home health background
- Bilingual (Spanish)
