Role Snapshot
- Role: Licensed Practical Nurse (Home Health)
- Location: Virginia Beach & Norfolk, VA • Onsite (home visits, local travel only)
- Pay: $48 per visit (~$70,000/year avg.)
- Schedule: Full-time, Monday–Friday, 8am–5pm • 5–6 visits/day
- Work with a stable, Medicare-certified home health team serving Hampton Roads. Clear routes, consistent workload, and full benefits.
What You’ll Do
- Conduct home health visits and basic assessments
- Deliver wound care, med administration, and skilled nursing
- Support start-of-care and follow-up visits (no OASIS lead)
- Coordinate with RN case managers and therapists
- Complete visit notes within 72 hours
- Communicate changes in patient condition promptly
Must-Haves
- Active LPN license in Virginia (good standing)
- 6+ months of home health experience
- Current CPR certification
- Valid driver’s license and reliable insured vehicle
- Must pass OIG, state, and national background checks
