What you’ll do
- Own your patient panel (no rushing)
- Manage chronic conditions + do full assessments
- Close care gaps (HEDIS and STAR measures)
- Lead care planning with your team
- Keep families informed and supported
Must-haves
- Active, unrestricted APN license in Ohio
- Board certified + Medicare credentialed
- 6+ months skilled nursing or long-term care experience
- Comfortable with digital tools + team-based care
- Willing and able to travel regionally as required
