We’re hiring a Registered Nurse Case Manager to manage patient care plans from start to finish. You’ll visit patients in their homes, help them recover or manage chronic conditions, and make sure everyone’s on the same page—from family members to doctors to PTs. No drama. Just nursing that actually makes a difference.
What you’ll do:
Coordinate care for a set group of home health patientsDo in-home assessments and create care plansEducate patients and families about medications, symptoms, and follow-upDocument accurately and on timeWork closely with aides, therapists, and doctorsKeep everyone in the loop
What you need:
RN license in NY, in good standing1+ year clinical experience (home health or med/surg preferred)Solid clinical judgmentAbility to manage your time without being micromanagedDriver’s license + reliable vehicle
