Location/Type:

Bay Area (Alameda, Contra Costa, San Mateo, Santa Clara) • Onsite/in-field • Travel between patient homes
Pay: $115,000–$125,000

Schedule:

Full-time • Monday–Friday • 40 hrs/week • On-call rotation

What you’ll do


  • Lead hospice patient care (home-based)
  • Complete assessments, build care plans, adjust as needed
  • Coordinate with MDs and interdisciplinary team (fast decisions)
  • Manage caseload with visits set in advance (dedicated scheduler)
  • Chart in HospiceMD (modern EMR)
  • Support families with education and counseling
  • Flex for urgent changes in patient condition (OT paid)

Must-haves


  • Active CA RN license
  • 2+ years nursing, with 1+ in hospice, home health, or SNF
  • Current CPR certification
  • Valid driver’s license + insured car (in good order)
  • Open to covering at least 2 Bay Area counties
  • Able to flex for on-call and after-hours
