Pay: $115,000–$125,000
Schedule:
Full-time • Monday–Friday • 40 hrs/week • On-call rotation
What you’ll do
- Lead hospice patient care (home-based)
- Complete assessments, build care plans, adjust as needed
- Coordinate with MDs and interdisciplinary team (fast decisions)
- Manage caseload with visits set in advance (dedicated scheduler)
- Chart in HospiceMD (modern EMR)
- Support families with education and counseling
- Flex for urgent changes in patient condition (OT paid)
Must-haves
- Active CA RN license
- 2+ years nursing, with 1+ in hospice, home health, or SNF
- Current CPR certification
- Valid driver’s license + insured car (in good order)
- Open to covering at least 2 Bay Area counties
- Able to flex for on-call and after-hours
