Hospice RN Case Manager — Glen Carbon, IL (Madison County)
$45.00/hour (flat) • Unhurried visits • Nurse-led care • Real career growth
If you became a nurse to truly care—teach a family, ease symptoms, sit and listen—this is that role. We design schedules so you can spend real time with patients, not race a stopwatch. You’ll join a team that prioritizes patient dignity, teammate happiness, and your long-term growth.
Why this will feel different
- Time at the bedside. Balanced caseloads and tight territories so you’re present with families—not living in your car.
- Your voice matters. Nurse-led care standards, responsive leaders, and IDT huddles that actually fix problems.
- Fewer late-night notes. Hospice-focused EMR (NetSmart or similar), clear documentation templates, and clinician-built workflows.
- Admissions support + on-call sanity. Coordinated admissions and optional dedicated on-call tracks help protect your evenings and headspace.
- Growth you can see. Structured ladders into preceptor, educator, supervisor, and leadership roles—plus protected training and hospice-specific CE.
