Job Overview:


You'll coordinate hospice care for patients and families across St. Louis County (excluding city limits). You'll manage your own caseload, develop care plans, conduct assessments, and handle clinical documentation using NetSmart (charting bedside with tablets). Regular collaboration with a supportive clinical team, including physicians, social workers, and hospice aides, will be part of your daily routine.

Key Responsibilities:


  • Assess patients regularly, create and update care plans.
  • Educate patients and families about hospice care.
  • Coordinate with physicians for patient care updates.
  • Complete clinical notes accurately and timely.
  • Provide compassionate end-of-life care and emotional support.
  • Actively participate in interdisciplinary meetings.
  • Ensure compliance with Medicare, Medicaid, and HIPAA regulations.
  • Handle occasional on-call responsibilities as needed.

Qualifications Needed:


  • Current Missouri RN license (unrestricted).
  • Minimum 1–2 years of RN experience required (hospice preferred).
  • Ability to work independently with minimal mentorship.
  • Strong clinical judgment, communication, and assessment skills.
  • Valid driver's license, reliable transportation.
  • EMR proficiency (NetSmart experience a plus).

Compensation & Schedule:


  • Competitive pay range: $37.48 - $47.12 per hour (based on experience) plus a $10K sign-on bonus!
  • Flexible schedules including evenings, weekends, and holidays as needed.
  • Separate On-call positions available to reduce staff nurse on-call load.
