We’re hiring a Registered Nurse Case Manager to manage patient care plans from start to finish. You’ll visit patients in their homes, help them recover or manage chronic conditions, and make sure everyone’s on the same page—from family members to doctors to PTs. No drama. Just nursing that actually makes a difference.
What you’ll do:
- Coordinate care for a set group of home health patients
- Do in-home assessments and create care plans
- Educate patients and families about medications, symptoms, and follow-up
- Document accurately and on time
- Work closely with aides, therapists, and doctors
- Keep everyone in the loop
What you need:
- RN license in NY, in good standing
- 1+ year clinical experience (home health or med/surg preferred)
- Solid clinical judgment
- Ability to manage your time without being micromanaged
- Driver’s license + reliable vehicle
