Health Care for the Homeless is seeking a strategic, compassionate, and experienced Director of Care Coordination to lead integrated, patient-centered care coordination efforts. In this essential role, you’ll guide a high-performing multidisciplinary team, manage value-based care programs, and deliver direct services that make a meaningful impact.
What You’ll Do:
- Lead and mentor a multidisciplinary team including RN Care Coordinators and Case Managers
- Oversee care coordination efforts across risk stratification, transitions of care, outreach, and chronic condition management
- Manage and drive performance of value-based care programs (including AHEAD, MDPCP, and other payer models)
- Partner with organizational leaders to integrate care strategies that support quality goals and whole-person care
- Deliver direct clinical services (30–40% of time) to complex patients needing care management and follow-up
- Leverage data analytics to monitor care gaps and improve patient outcomes and system efficiency
What You’ll Bring:
- Active RN or NP license in Maryland (required)
- Bachelor’s degree in health care or related field (Master’s preferred – MSN, MPH)
- Minimum 5 years’ experience in care coordination, case management, or population health
- At least 3 years’ experience managing staff in a multidisciplinary clinical setting
- Experience with value-based care models (AHEAD, MDPCP, ACOs) and EHR/care management systems
- Strong critical thinking, leadership, communication, and collaboration skills
