Director of Care Coordination

What You’ll Do:

• Lead and mentor a multidisciplinary team including RN Care Coordinators and Case Managers
• Oversee care coordination efforts across risk stratification, transitions of care, outreach, and chronic condition management
• Manage and drive performance of value-based care programs (including AHEAD, MDPCP, and other payer models)
• Partner with organizational leaders to integrate care strategies that support quality goals and whole-person care
• Deliver direct clinical services (30–40% of time) to complex patients needing care management and follow-up
• Leverage data analytics to monitor care gaps and improve patient outcomes and system efficiency

What You’ll Bring:

• Active RN or NP license in Maryland (required)
• Bachelor’s degree in health care or related field (Master’s preferred – MSN, MPH)
• Minimum 5 years’ experience in care coordination, case management, or population health
• At least 3 years’ experience managing staff in a multidisciplinary clinical setting
• Experience with value-based care models (AHEAD, MDPCP, ACOs) and EHR/care management systems
• Strong critical thinking, leadership, communication, and collaboration skills
