Health Care for the Homeless as a SNAPT NP Fellow, a one-year, full-time fellowship offering a unique opportunity to provide high-impact, trauma-informed care to individuals and families experiencing homelessness—both in our clinic and through outreach in the community.
What You’ll Do:
- Deliver comprehensive, patient-centered care including assessment, diagnosis, treatment, and referral
- Provide preventive, chronic, and acute care services in clinical and community-based settings
- Collaborate within a multidisciplinary team to support integrated treatment planning
- Engage in outreach efforts that reduce barriers to care for underserved populations
- Participate in a structured fellowship curriculum with mentorship and professional development
- Actively contribute to racial equity initiatives and address health disparities in practice
What You’ll Bring:
- Master’s or Doctorate in Nursing with Maryland licensure as an Adult or Family Nurse Practitioner
- CDS and DEA licensure (with DATA 2000 waiver)
- BLS CPR certification
- Experience or strong interest in serving individuals experiencing homelessness, poverty, and health disparities
- Strong communication, collaboration, and cultural humility
- Commitment to trauma-informed care and continuous learning
