ACP Emergency Department/Inpatient Navigator
General Job Summary:
The ACP Emergency Department / Inpatient Navigator will be responsible for helping patients navigate the complex healthcare environment, ensuring they receive timely, appropriate, and comprehensive care. They act as a patient advocate, connecting patients with resources, providing education, and coordinating care with hospital staff to improve patient outcomes and satisfaction.
Primary Job Responsibilities:
- Identify and round on TMP patients at Presbyterian Main and Matthews Emergency and inpatient facilities.
- Collaborate with ED physicians to evaluate potential for hospital admission and discuss appropriate alternatives (e.g., observation, home-based follow-up, referral back to PCP).
- Engage with patients in person and virtually to explain care plan next steps and serve as a liaison between hospital-based teams and TMP primary care physicians. This includes coordinating with Tryon’s internal teams (care management and scheduling) to support seamless transitions.
- Document non-clinical encounters and care coordination actions within the EMR and provide updates to TMP PCPs regarding patient status and ED/inpatient events.
- Serve as TMP’s primary point of contact with Novant ED/inpatient care teams. This includes attendance at ED daily huddles both in-person and/or virtually.

Requirements:
Education and Certifications:
- Completion of an accredited physician assistant or nurse practitioner program with appropriate certification exams.
- Has and maintains current Physician Assistant license with the North Carolina Medical Board or Nurse Practitioner license with the North Carolina Board of Nursing.
- Must have or be eligible for a DEA license.
- Eligible for prescriptive authority application through the Board of Nursing or Medical Board in NC required.
- BLS for healthcare provider from AHA or American Red Cross required with expiration date greater than 90 days from date of beginning practice.
- Completion of Hepatitis B Vaccine and Flu Vaccine Form will be required. Candidate may decline vaccination through declination form or may provide record of vaccination from previous employer.
- Completion of TB test will be required.
Experience
:
- 1+ year of experience is preferred.
- Experience in Emergency Department or hospital setting.
- Experience with Athena and Epic EMRs a plus.
- Excellent communication skills.
Physical Requirements:
- Work consistently requires walking, standing, sitting, lifting, reaching, stooping, bending, pushing, and pulling.
- Must be able to lift and support weight of 35 pounds
- Ability to concentrate on details.
- Use of computer for long periods of time.
