Homelessness is the largest social and public health crisis in California. Illumination Health + Home  is a growing non-profit organization dedicated towards disrupting the cycle of homelessness by providing targeted, interdisciplinary services in our recuperative care centers, emergency shelters, housing services and children's and family programs. IHH currently has 13+ facilities with 22+ micro-communities scattered across Orange County, Los Angeles County and the Inland Empire.

Job Description


The Referral Coordinator plays a critical role in the intake and coordination process for homeless clients referred to from hospitals and other medical or social service agencies. This position ensures that all incoming referrals meet admission criteria for the Recuperative Care program, coordinates with healthcare providers for client discharges, and maintains compliance with all contract requirements. The Referral Coordinator serves as a liaison between Illumination Health + Home and referring entities across multiple counties to facilitate seamless transitions into care.

Additionally, the role includes administrative responsibilities to support accurate tracking, reporting, and billing.  The pay range for this role $20-$22 per hour. The Schedule for this role will be Monday/Friday, 2:00pm-7:00pm, Wednesday/Thursday, 12:30pm-9:00pm. 

Responsibilities


Referral Management & Evaluation 


  • Review incoming referrals from hospitals, Managed Care Organizations (MCOs), Independent Physician Associations (IPAs), and other agencies. 

  • Evaluate each referral for eligibility against program admission criteria. 

  • Collect and verify preliminary referral information to ensure completeness. 

  • Coordinate directly with hospital, IPA, or MCO contacts for required documentation or clarification. 

  • Maintain communication with referring entities throughout the intake process. 

Discharge & Transition Coordination 


  • Work with hospital and agency discharge planners to coordinate the timely and appropriate transition of eligible clients to the Recuperative Care Center. 

  • Ensure all client intake information is received prior to arrival and shared with the appropriate internal staff for onboarding. 

  • Provide consistent updates to stakeholders to ensure alignment during the discharge planning process. 

Documentation & Reporting 


  • Maintain accurate referral logs and tracking spreadsheets. 

  • Prepare monthly summaries and data reports in alignment with contract requirements. 

  • Complete and submit referral forms required for hospital billing. 

  • Maintain records that are audit-ready and in compliance with Recuperative Care program standards. 

Stakeholder Engagement & Communication 


  • Follow up with hospitals and other referral sources regarding pending or incomplete referrals. 

  • Schedule and participate in in-service meetings or training sessions with referral partners to ensure program understanding and process alignment. 

  • Build and maintain professional relationships with hospital personnel, case managers, and community agency representatives. 

Preferred Experience/ Minimum Qualifications


Required:

  • High School Diploma or GED 

  • Minimum of one (1) year of experience in case coordination, patient navigation, medical administrative support, or a related field 

  • Strong organizational skills with attention to detail 

  • Proficiency in Microsoft Office Suite (Outlook, Word, Excel) 

  • Excellent written and verbal communication skills 

  • Ability to manage time effectively in a fast-paced, multi-agency environment 

Preferred

:


  • Associate or bachelor’s degree in public health, Healthcare Administration, Human Services, or a related field 

  • Experience working with homeless or medically vulnerable populations 

  • Familiarity with Managed Care Organizations (MCOs), IPAs, or hospital discharge procedures 

  • Experience using electronic health record (EHR) systems or referral tracking tools 

  • Bilingual in English and Spanish
