Nurse Practitioner – Ashtabula, OH

We’re looking for a Nurse Practitioner to care for nursing home patients in Ashtabula. If you’ve got clinical experience and want to work smarter—not harder—this role is for you.
What You’ll Do:
- Provide primary care to a steady panel of patients
- Lead on-site care planning with a “treat in place” approach
- Do assessments, write care plans, coordinate with teams
- Help close care gaps like HEDIS and STARs
- Handle prior auths, med reviews, and chronic care management
- Talk with families and teams so everyone’s on the same page
What You Need:
- Active NP license in Ohio + national certification
- Medicare participation eligibility
- 6+ months as an NP (geriatric or palliative care is a plus)
- Experience with chronic conditions and nursing home care
- Strong communication + tech comfort
