Certified Diabetic Educator


Job Summary:


A diabetes education specialist must be able to give extensive advice on how patients with diabetes can improve their health. These professionals should be able to inform patients about diabetes and the steps to manage the disease. Diabetes education specialist will continue to work with patients to develop long-term strategies for maintaining a healthy diet, monitoring blood glucose, taking medications, and increasing physical activity. These professionals will teach patients how to use appropriate equipment to treat and monitor diabetes including blood glucose monitors, continuous glucose monitors, medication delivery devices, and insulin pumps.(This is a full-time position that will support our Value Based Medicine team at Southpark, Monday to Friday 8 am to 5 pm)

Primary Job Responsibilities/Tasks may include, but not limited to:  


  • Teaching patients about diabetes.
  • Teaching patients how to cope with disease.
  • Working with patients to develop long-term strategies for maintaining a healthy diet, monitoring blood glucose, taking medications, and increasing physical activity
  • Teaching patients how to use appropriate equipment, such as insulin pumps, continuous glucose monitors, and blood glucose meters, to treat and monitor diabetes.
  • Informing patients about wise food choices.
  • Keeping track of patients’ blood glucose levels over time and notating which medications provided the best results.
  • Support Value Based Medicine (VBM) specific strategies to develop and support workflows.
  • Other duties as assigned.

Requirements:


Education and Certifications

  • High school diploma. Degree in nursing, nutrition, pharmacy, occupational therapy or social work.
  • Must be certified diabetes educator.
  • Completion of Hepatitis B Vaccine and Flu Vaccine Form will be required.  Candidate may decline vaccination through declination form or may provide record of vaccination from previous employer.
  • Completion of TB test required.

Experience


  • 2+ years working with patients with diabetes, 1,000 hours of diabetes self-management education required to become certified diabetes educator, and a minimum of 15 hours of continuing education coursework annually related to diabetes treatment and care.

Physical Requirements


  • Work consistently requires walking, standing, sitting, lifting, reaching, stooping, bending, pushing, and pulling.
  • Must be able to lift and support the weight of 35 pounds.
  • Ability to concentrate on details.
  • Use of computer for long periods of time.
