Job Title: Clinical Therapist


Department: Integrated Health


Reports To: Program Manager of Clinical Services


FLSA Status: Exempt


Summary:


The clinical therapist will provide mental health therapy services to youth/children and adults with Intellectual/Developmental Disabilities (IDD) within an integrated health clinic. This position requires supporting the case manager in the formulation of an individualized treatment plan which will be used in conjunction with the entire primary care team to develop a holistic care strategy for each client.

Requirements and Duties:


  • Organizing Workload includes prioritizing work assignments in a manner consistent with consumer and program needs while making the most efficient use of available work time
  • Provide psychotherapeutic support/counseling to client according to DSM 5 guidelines 
  • Discuss continuing care needs with multidisciplinary care team.
  • Ensures the proper handling and compliance of confidential medical records (EMR)
  • Maintains record request for Medical records from other sources.
  • Follow-up planning for clients with post-acute transitional concerns 
  • Document assessment findings in SW outpatient assessment or SW progress notes within EMR
  • Communicate material findings and tentative transition plan with care team (nurse, physician, case management etc.)
  • Documentation includes complete, accurate and timely completion of clinical and statistical records of staff activities.
  • Documents every consumer/guardian  and collateral contact in compliance with agency policy.
  • Complete various administrative forms to assure accurate data collection for billing, fee setting, the management information system and other purposes in accordance with agency policy.           

Requirements:


Certificates, Licenses, Education:


  • Education: MSW Master of Social Work 
  • Required Licensure: LMSW,LMFT,LPC,LLMSW,LLMFT,LLPC

Hybrid Work Policy:


This position offers a hybrid work schedule contingent on meeting productivity and telehealth visit standards established by leadership and may be adjusted at any time based on the agency's needs. Employees may work remotely for a portion of their schedule; however, hybrid work does not permit full days working from home if clients are requesting in-person appointments in the office or community. Staff are expected to be flexible and prioritize client needs, ensuring timely and appropriate service delivery.During the first 30 days of employment, staff are required to work in the office.

This period may be extended based on performance..
