Our client is a well-established leader in hospice care and is searching for dedicated RN Case Managers to join their compassionate team and make an impact in the community.
What You’ll Do and Achieve
- Deliver Heartfelt Care: Provide high-quality, patient-centered care focusing on the physical, emotional, and spiritual well-being of each patient. You’ll complete 20-25 visits per week and manage a caseload of 11-13 patients.
- Be a Guiding Presence for Families: Offer families reassurance and expert support as they navigate care journeys, ensuring they feel informed and empowered.
- Experience True Independence: Manage your caseload with autonomy while being fully supported by a collaborative and compassionate team.
- Champion Dignity: Ensure each patient’s comfort, preferences, and dignity are respected and prioritized at every stage of care.
What You’ll Bring
- 1+ Year(s) of Hospice Experience: Your experience and expertise in hospice and palliative care will help patients and families find peace and comfort.
- Commitment to Forth Worth (Tarrant County): Must be willing to provide care in Tarrant County.
