Make a Real Impact as a Hospice RN Case Manager


Full-Time | Mon–Fri | 8am–5pm | Light Backup Call Rotation


We’re hiring full-time RN Case Managers for a top hospice and palliative care provider serving Twin Falls and the surrounding area. If you want a role where you have independence, strong team support, and the space to truly care—this is it.

What You’ll Do


  • See 20–25 visits per week and manage a caseload of 11–13 patients
  • Deliver patient-centered hospice care across Twin Falls
  • Support families with guidance, education, and presence
  • Work independently while staying connected to a collaborative care team
  • Focus on comfort, dignity, and quality of life

Schedule


  • Full-time: Monday through Friday, 8:00am–5:00pm
  • One evening backup call per week (5:00pm–8:00pm)
  • Weekend backup on-call rotation (every 11 weeks)

What You’ll Bring


  • RN license and at least 1 year of hospice experience
  • Ability to travel locally around the Twin Falls area
  • Confidence, empathy, and strong clinical judgment
