Our client is looking for an RN Case Manager to provide field-based hospice care in Victoria, Texas. This role is part of a larger, experienced hospice network with a proven ability to scale care teams and grow in new markets. They're looking for someone reliable, compassionate, and ready to make a difference—without being afraid to be hands-on.

What You’ll Do:


  • Deliver compassionate hospice care in the field, managing a caseload of patients.
  • Coordinate and communicate with the interdisciplinary care team, patients, and families.
  • Ensure compliance with hospice standards and maintain thorough, timely documentation.
  • Be available for on-call shifts (with extra pay) and occasional after-hours visits (also paid per visit).
  • Play a key part in providing stability and continuity of care to the community.

What You Need:


  • RN license in Texas (required).
  • Hospice experience preferred, but not required—training is available for nurses with the right mindset.
  • Solid clinical skills and comfort with fieldwork.
  • High level of dedication, compassion, and reliability.
  • Willingness to take part in on-call rotation and handle patient visits as needed.
