Our client is a well-established leader in hospice care, seeking two dedicated RN Case Managers to provide compassionate, patient-centered care that truly matters.
What You’ll Do
- Deliver High-Quality Care – Provide comprehensive, patient-focused care, emphasizing physical, emotional, and spiritual well-being. You’ll manage a caseload of 11-13 patients and complete 20-25 visits per week.
- Support Families – Offer guidance and emotional support to families, helping them navigate care decisions with confidence.
- Autonomous Caseload Management – Work independently while backed by a collaborative, supportive team.
- Uphold Patient Dignity – Respect and prioritize each patient’s comfort, preferences, and dignity at every stage.
What You Need
- 1+ Year of Hospice Experience – Bring your expertise in hospice and palliative care to provide comfort and peace to patients and families.
- Service Area Commitment – Must be willing to provide care throughout Collin County, Denton County, and North Dallas.
- HomeCare HomeBase Experience (Preferred, Not Required) – Familiarity with this EMR system is a plus but not mandatory.
