Registered Nurse (Full-Time or Part-Time) – Home Health

What You’ll Do:
- Assess patients’ physical, emotional, and social needs.
- Develop and update care plans with patients and caregivers.
- Provide direct nursing care, including wound care, infusion therapy, and medication management.
- Educate patients and caregivers to help them regain independence.
- Monitor patient progress and communicate updates to medical providers.
- Supervise and guide LPNs and HHAs in patient care.
- Keep accurate, timely records of care plans, progress, and treatment.
- Participate in on-call, weekend, and holiday rotations as needed.
- Stay current with nursing best practices through ongoing education.
What You Need:
- Active NY State RN license.
- At least 1 year of recent geriatric care experience.
- Home care experience is a plus.
