About the role:


As a Value Based Care (VBC) Medical Record Specialist for Accompany Health (AH) you will be: Performing external medical record note review to populate pertinent historical information in the patient’s Accompany Health (AH) Athenahealth record.

Responsibilities will include:


  • Review collected medical records housed in a repository, through the athenahealth  Electronic Medical Record (EMR) or via EMR access to outside facility records, for pre-visit chart prep.
  • While reviewing the above medical records, identify key evidence for quality gap closure and transfer that data to designated locations within the patient’s AH athenahealth medical record.
  • Identify and transfer pertinent lab results to populate the AH athenahealth medical record.
  • Transfer Medication List to the AH athenahealth medical record.
  • Summarize identified trends with each patient (i.e., high ED use, frequent hospitalizations, multiple doses of same/similar medication and/or multiple pharmacies, etc.), and communicate findings to the clinician via EMR notes.
  • Categorize key documents into appropriate chart note sections for clinician  ease of review. 
  • Other duties as assigned.
  • This role reports to the Sr. Director VBC Operations.

Desired skills and experience:


  • Required
  • Associates degree, Medical Record Certification (i.e. AHIMA), or equivalent
  • 3+ years of relevant work experience in healthcare industry with health information and/or direct medical record review and oversight
  • Comfort with multiple technology platforms, including but not limited to a variety of EMRs
  • Attention to detail 
  • Preferred 
  • Experience in pre-visit planning 
  • Experience with athenahealth
  • Experience with GSuite and Google applications

$20 - $20 an hourThe US base salary range for this full-time position is $20/hour + equity + benefits.  Our salary ranges are determined by role, level, and location.  The range displayed on each job posting reflects the minimum and maximum target for new hire salaries for the position.  Within the range, individual pay is determined by work location and additional factors, including job-related skills, experience, and relevant education or training.  Our talent team can share more about the specific salary range for your preferred location during the hiring process.#LI-Remote#LI-MP1For Patient Facing RolesTo keep our patients, communities and each other safe, you'll be required to comply with Accompany Health’s medical clearance requirements, including completing a TB screen and providing proof of immunity or vaccination for certain conditions.
