QUALIFICATIONS/REQUIREMENTS: 


Education:  Masters in Behavioral Health Discipline Previous experience/work history:  At least three years of experience in the provision of behavioral health treatment and/or rehabilitationLicensing/Certification:  PA Licensure in a behavioral health discipline; CPRP eligibleDriver’s License/Use of own vehicle is Required 

JOB SUMMARY:


To promote the mission of PATH to help individuals achieve a more independent and fulfilling life by managing an effective team to provide quality services.

SPECIFIC DUTIES:


1.         Effectively supervise a Core Support Team within the Renaissance Program2.         Maintain team compliance with DPW’s Psychiatric Rehabilitation Medical Necessity Criteria and Standards and Philadelphia DBH requirements3.         Interview and assign individuals referred to Renaissance as requested by the Program Director4.         Monitor service provision to ensure that each individual receives quality, effective and appropriate clinical services  5.         Ensure individuals gain access to needed psychiatric, medical, social, residential, financial and other services appropriate for the individual6.         Ensure active engagement of individuals served including outreach visits at the individual’s home, crisis center, or inpatient facility or other appropriate locations7.         Ensure all individuals assigned to team develop a psychiatric rehabilitation plan and receive rehabilitation services utilizing the psychiatric rehabilitation approach developed by Boston University8.         Ensure provision of individualized support and skill teaching in the community to promote new role development9.         Ensure that all psychoeducational, self-help and adjunctive activities identified by the Stakeholder Committee are reviewed and implemented as assigned by the Program Director10.      Consult with the Specialized Treatment Team Program Manager and psychiatrists on a regular basis to coordinate appropriate supports for each individual11.      Oversee and support participants in the Renaissance Canteen Operation12.      Ensure approved cases are discharged in a timely manner13.      Monitor Renaissance service area to ensure health and safety of Renaissance participants14.      Ensure Renaissance staff availability 24 hours/7days a week to provide crisis coverage for all Renaissance participants which includes responding to phone calls within 15 minutes and providing face-to-  face interventions indicated to resolve a crisis or respond to a participant or family member’s request for face-to-face service15.      Participate in the provision of 24 hour/7 day a week supervisory coverage for on-call staff as assigned  16.      Meet productivity standards for community integration hours 17.      Provide direct service as indicated to meet individual and program needs18.      Appropriately schedule and ensure appropriate usage of program vehicles19.      Serve as an effective member of the Renaissance Stakeholder Committee20.      Ensure collaboration with involved personnel in the community such as GED instructors, college professors, faith-based representatives, volunteer and employment supervisors21.      Carefully review, follow up, add appropriate clarification and process all incident reports generated by your team in a timely fashion22.      Carefully review, follow up, correct and process all BH Review Committee Minutes generated by your team in a timely fashion23.      Participate in the development of proposals as requested

Life at PATH (People Acting To Help)

Our roots can be traced back to 1973 when PATH was incorporated as a not-for-profit corporation. This came about from the efforts of a group of dedicated community members, many of whom had family members in need of services, which were scarce to non-existent at the time. They worked tirelessly with a group of caring professionals to form PATH and to secure funding in order to provide much needed services to the citizens of Northeast Philadelphia. This tradition of the involvement of dedicated and compassionate community members continues today through our volunteer Board of Directors who are all residents of Northeast Philadelphia. The Board of Directors, which is largely composed of family members of individuals who participate in our programs, continues to strongly advocate for the needs of the community. Over the years, our community has expanded to include individuals throughout the city of Philadelphia and in surrounding counties through our Specialized Russian services Starting with a staff of 14 in rented office space providing only information and referral services, PATH has significantly expanded its programs as we currently serve over 3,000 individuals annually in our various Behavioral Health Programs, which include both Mental Health and Substance Use Treatment, as well as in our Intellectual Disabilities programs. With over 500 competent and considerate staff members, we are able to provide a wide array of quality services to meet individuals' needs. Our facilities include our main campus at 8220 Castor Avenue, 4 satellite locations, and over 70 homes in the community where we provide residential supports to enable individuals to remain living in the community. We also provide school based services in numerous Philadelphia schools. We have remained true to our mission of helping individuals achieve a more independent and fulfilling life.
